Emergency Consent Forms

If you are sending over an emergency patient and they will be choosing IV or Oral Sedation and we ask you to have them sign their oral surgery consent prior to taking their oral sedation medication (such as Valium/Halcion) please select and print out the appropriate form below. (Patient’s cannot sign legal documents with sedation on board) A spouse or family member may do this for them here in our office otherwise. Thank you!
