Dr. Catalano is an in-network provider with most major Dental Insurance companies such as: Metlife, Delta Dental, Anthem/BCBS Dental, United Concordia Dental & Cigna Dental to name a few. “(Please note these are subject to change and each individual policy has different coverage details.)
We do not accept medical insurance or Medicare/Medicaid, however …
For those with Medicare/Medicaid or, no in-network benefits, still hoping to have Dr. Catalano as their oral surgeon, please request a discounted-self pay estimate. Dr. Catalano is very accommodating to all his patients with no insurance benefits to make their oral surgery needs affordable for them. We are here to help! Our office will offer in-house payment plans coming soon!
The box/button below Insurance Options (with the aspen leaves) has some other insurance companies that our office currently accepts. We recommend you call your member phone number on your dental insurance plan and ask if Dr. Catalano is an in-network provider to confirm.
At your initial new-patient office visit appointment, Dr. Catalano will assess the condition/procedure you are inquiring on, and our office will provide you with an estimate of future procedure charges/estimated patient portions. Any X-Rays or 3-D scans taken the date of your office visit, or exam fees and are not covered/not eligible by your insurance will be your patient responsibility and payable at the time of your office visit/procedure.
Payment is due on or before the time services are rendered, unless other financial arrangements have been made in advance. We are here to answer any questions you may have regarding your procedure, payment, or insurance.

For your convenience, we accept Visa, Mastercard, Discover, and American Express. We no longer accept Care Credit. Personal checks are accepted once verified, not to exceed more than $500.00 per visit. We do accept verified money orders as well.
